Your road to successful communication and/or swallowing begins with an evaluation. A therapist will evaluate and identify your needs. The evaluator will then design an appropriate treatment program to help you meet your communication goals.
Scheduling an evaluation is easy. We first require a referral from your primary care provider. You can ask them to fill out this prescription. We will contact you after we receive this form to schedule an appointment.
Before your initial appointment, please click on the appropriate paperwork page (pediatric or adult) to download, print, and fill out paperwork before your appointment. Please show up 15 minutes early with your paperwork already filled out.The Patient Handbook provides helpful information about our policies and procedures.
Your first appointment will consist of an evaluation by a licensed and ASHA certified Speech-Language Pathologist. Areas being evaluated may include any or all of the following: speech, language, voice, fluency, swallowing, feeding, oral-motor, cognition, reading, handwriting. After the evaluation, the therapist will review findings and whether or not therapy is warranted. The evaluation report will by typed up and sent to the primary care provider for review and approval. From that point on, therapy (if warranted) will be set up for a consistent basis (weekly or biweekly usually).
